Check-up Tickets for National Health Insurance Certificate Holders

Tokutei Kenko Shinsa (Health Check-up for Specific Purpose) is going to be offered for adults between the ages of 40 to 74, who received National Health Insurance benefits before April 1.
There are check-ups for high blood pressure, hyperlipidemia, diabetes, and other adult diseases, and there will be support to improve the symptoms or advice to prevent such problems. Tickets, questionnaires, and a list of facilities are sent to eligible people twice this year. The first set was sent in May, to those born between April and November, and the second set will be sent at the beginning of August, for birthdates between December and March.
Fee: \1,200 (if you are exempt from city-tax in 2009, then the fee is \400.)
To Apply: Make an appointment ahead of time at the facility of your choice, selected from the list provided; you must bring the check-up ticket, the questionnaire and your most recent National Health Insurance Certificate.

Application for Housing Units for Elderly People

In Aoba-ku, a new special nursing-care home will be open in February, 2011. There will be a lottery held. The units are available for those with certified nursing care levels 1 to 5. Payment of 10% of the nursing care cost and a few other costs are required. Notices will be sent after October, to those who win the lottery.

Subsidies for Children and Disabled Persons

National Pension Plan Payment: Exemption and Deferment of Payment

Eligibility: Persons whose previous year’s income was less than a determined amount or those who have difficulties making national pension plan payments due to unemployment etc.
To Apply: Fill in an application form available at your local ward office’s Kokumin nenkin-kakari, National Pension Plan Section and submit it at the same place. At the time of application, please bring ①National Pension Plan Handbook ②Seal (Unnecessary if the applicant is providing his or her own signature) ③Those who have moved from other cities, please bring documents showing the previous year’s income. ④If unemployment is the reason for application, please bring an unemployment slip, etc.

Kodomo Teate (The New Child Allowance)

This new allowance will be provided to children until the last day of the school year (March 31) during which the child reaches the age of 15.
Recipients: Mothers or fathers of the eligible children, or their legal caretakers
Amount: \13,000 per child
Income Restrictions: None
Payment months: Three times a year; ①Jun, 2010 (for Apr and May), ②Oct, 2010 (for Jun thru Sep) and ③Feb, 2011 (for Oct thru Jan, 2011) From 2011 onwards, it will be; ①Jun (for Feb, thru May), ②Oct (for Jun thru Sep) and ③Feb (for Oct thru Jan)
Applicants: Applicants should be the person in the family who is the children’s main financial supporter.
To Apply: Application forms are being sent to the eligible families. Please fill them in, and return the forms to apply. In order to receive the allowance from June, you should apply by the end of May. However, if you apply by the end of September, you may receive the sum from June. You cannot receive the past payments if your application is later than October.

Non-Japanese caretakers of the eligible children should meet the following requirements to be able to apply:
・Should have registered as a foreigner in Yokohama.

Kindergarten Tuition Subsidies

Subsidies of between \48,000 and \220,000 (depending on your residential tax submitted for 2010) will be provided to families with children who have enrolled in a private kindergarten between Apr.1 and Jun.1. (For children enrolled in a private kindergarten after Jun.2, please apply in Nov. 2010, or Feb. 2011.) The forms will be distributed at the end of Jun. to each kindergarten and may be submitted there.

Application for Public Housing for Elderly People

There will be a lottery held for placement on a primary waiting list to fill vacant apartments. These apartments are designed to provide a safe environment for senior citizens over 60, including barrier-free amenities and emergency call services. *There is no nursing care. *Housing will be available in the case of a vacancy between Aug.2010 and Jul.2011. *An income-dependent rent subsidy is available.
Eligibility: People who are living or working in Yokohama at the time of application. Age restrictions do apply.
